January is Open Enrollment, so any members who wish to make any changes to their benefits package, this is the month to make those changes! 

We are happy to announce that our Health and Welfare trustees were successful in improving the screening offered by your Personal Physician.  Our members will not lose any health exam benefits and will still earn $200.00    by completing their yearly wellness exam for members and their spouses, as long as they are covered under our insurance program.
You will need to complete the new “2024” Physician Wellness Screening Form located on our website or contact our office and we will be happy to mail you a copy.

All members have received their temporary cards, and all final cards will be mailed out within 7-10 days from Wellmark. The DC 4 Office will mail out informational packets of the available services to all participant members in the upcoming weeks.
