Underneath it states "Find a doctor or facility". Click on the arrow.

Click on United Healthcare ChoicePlus (you may receive a prompt, that you are leaving the website)

Next, you can enter your home zip code or your doctor's zip code.

Next, a search bar will appear, and you will enter your doctor's name here.
